Evidence required
Certificates issued by a Government Blood Bank, Medical College Blood Bank, District Hospital Blood Bank, or any hospital licensed by the State Blood Transfusion Council.
Requirement
A valid certificate is required as proof. Only your highest achievement level counts toward activity points.
